Meet Our Founder
Dr. John M. Porter, O.D. obtained his pre-optometry degree with honors at Ohio University in Athens and is a graduate of The Ohio State University College of Optometry.
He began his career at a large ophthalmology group practice where he developed a strong interest in the evaluation and treatment of ocular disorders.
Since 1996, Dr. Porter has focused primarily on geriatric eye care and found it so professionally rewarding that he opened his mobile eye care practice, ElderCare Vision Services in 1999.

According to the American Optometric Association, 25 to 50% of skilled nursing facility residents have at least one vision impairment including cataracts, age-related macular degeneration, glaucoma, or diabetic retinopathy.
Sadly, most of them will never have an eye exam after their admission. Few will have optimal eye care.
That's why ElderCare was founded -- to provide the most caring and comprehensive mobile eye care available.
ElderCare Practice Highlights
Contract
- State certification: Copies of medical license, proof of insurance, annual, physical, and Mantoux tests available
- We put our mission and responsibilities in writing
- We maintain Medicare and HIPAA policies
Availability
- Visits are scheduled according to your needs (most facilities are visited monthly)
- You are always available to reach out to us via telephone, fax, or mobile phone
- 24-hour emergency consultation
- Personal eyewear dispensing is done as products arrive
Correspondence
- Patient schedules are provided about a week prior to the visit for changes/approval
- Visit summaries and word-processed exam forms are provided at the time of the visit
- Exam forms: MDS vision level and eyewear order information clearly stated action required by staff section (Rx required, orders written)
- Dispensing notification
- Referral forms
Optical Goods and Services
- Full 2-year eyeglass warranty for breakage, theft, or loss
- Written quotes and RP letters to explain recommendations
- Repairs/adjustments are free of charge (with some exceptions)
- Ample frame choice/universal pricing
- Competitive private spectacle prices (Rx SV average $120.00/Bi-focal Rx average $150.00)
- Low vision aides (magnifiers, telescopes, etc.)
- Accessories (straps, Sunwear)
- Engraving is free of charge on all items dispensed
Facility Promotion
- Personalized family letters and enrollment forms
- In-services for nurses and staff
- Health fairs
Patient and Family Services
- We always obtain responsible party approval for all non-emergent referrals and private pay optical goods
- Family attendance for exams and office calls are accepted and encouraged
- Our office is always available for questions about resident care and billing
